We are seeking a compassionate and organised Support Lead to oversee the coordination and delivery of our disability mentoring and peer support services.

Key Responsibilities

  • Coordinate referrals and match clients with appropriate mentors and support volunteers.
  • Manage client onboarding, support plans, and ongoing communication.
  • Oversee the day-to-day delivery of disability mentoring and companionship services.
  • Support and supervise mentors, ensuring they have the guidance and resources they need.
  • Schedule appointments, mentoring sessions, and follow-up reviews.
  • Monitor client progress and maintain accurate, confidential records.
  • Respond to enquiries and provide a welcoming first point of contact for clients and families.
  • Work closely with external organisations, community partners, and healthcare professionals where appropriate.
  • Ensure safeguarding, confidentiality, and organisational policies are followed.
  • Identify opportunities to improve service quality and client outcomes.

Skills and Experience

  • Experience coordinating disability, health, social care, or community support services.
  • Excellent organisational and communication skills.
  • Ability to manage multiple clients and priorities effectively.
  • Understanding of disability inclusion and person-centred support.
  • Experience working with people with lived experience of disability is highly desirable.
  • Knowledge of safeguarding, confidentiality, and professional boundaries.
  • Compassionate, empathetic, and committed to empowering others.
