We are seeking a compassionate and organised Support Lead to oversee the coordination and delivery of our disability mentoring and peer support services.
Key Responsibilities
- Coordinate referrals and match clients with appropriate mentors and support volunteers.
- Manage client onboarding, support plans, and ongoing communication.
- Oversee the day-to-day delivery of disability mentoring and companionship services.
- Support and supervise mentors, ensuring they have the guidance and resources they need.
- Schedule appointments, mentoring sessions, and follow-up reviews.
- Monitor client progress and maintain accurate, confidential records.
- Respond to enquiries and provide a welcoming first point of contact for clients and families.
- Work closely with external organisations, community partners, and healthcare professionals where appropriate.
- Ensure safeguarding, confidentiality, and organisational policies are followed.
- Identify opportunities to improve service quality and client outcomes.
Skills and Experience
- Experience coordinating disability, health, social care, or community support services.
- Excellent organisational and communication skills.
- Ability to manage multiple clients and priorities effectively.
- Understanding of disability inclusion and person-centred support.
- Experience working with people with lived experience of disability is highly desirable.
- Knowledge of safeguarding, confidentiality, and professional boundaries.
- Compassionate, empathetic, and committed to empowering others.
